The HUD-VASH Case Manager would be expected to conduct home visits, complete treatment plan, document all efforts in Clinical Patient Record System (CPRS), provide coaching, skills training, budget planning, service coordination and other supportive services as required by Veteran and his or her family; in an effort to achieve the treatment goals set by Veteran and Case Manager.
The above services shall be fulfilled in accordance with all terms, conditions, provisions, and requirements listed herein. The period of performance for these services is for a period of twelve months from date of award plus four (4) one-year option renewals exercisable at the government s sole discretions in accordance with 52.217-9. Please be aware that the Government intends to use a minimum of one social service provider. This is for 1 full time community-based case manager, 40 hour per week, with schedule (tour of duty) between 7am and 6pm. The schedule is designed to meet the needs of the Veterans; therefore, some weekends might be needed.
BACKGROUND INFORMATION
Homelessness, a significant national problem, has many causes. Individuals suffering homelessness are usually unemployed, unable to work, or have such a low income that they cannot access safe affordable housing. Some individuals who are homeless have disabling mental health, substance abuse, or physical conditions that lead to, or compound, their homeless situation. Families undergoing homelessness are seen more frequently than in the past when single individuals were the primary. In an effort to support the VA s Zero Tolerance on homelessness among Veterans, DC VAMC is seeking onsite masters level Case Manager to coordinate treatment services with VA and or community-based providers/ programs to meet the goal and needs of the Veteran and his or her family in HUD VASH Community Based Housing.
